Has Medicaid in the state of Indiana
 
Participates in the Health & Wellness Waiver or Pathways Waiver

Requires help with Activities of Daily Living 

  • Bathing

  • Dressing 

  • Toileting

  • Transferring (getting in and out of bed or chair)

  • Eating

  • Mobility​

     

Who can be a caregiver?

Must be age of 18 or older
Must live in the same household as the patient
Must participate in education provided by a Caregiving Coach
Must pass a background check
